29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Scotchman Punch - Employee punching holes with the Scotchman model 12012 in angle iron were exposed to amputation hazards from an unguarded point of operation, on or about June 29th 2018. b) Niagara Shear Press - Employees working near the Niagara Shear were exposed to point of operation hazards from an unguarded shear, on or about June 29th 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) Shear and Cold Saw Panel - Employees cycling power for the Cold Saw were exposed to electrocution hazards from the cover missing on the Shear and Cold Saw service panel, on or about June 29th 2018. b) Upstairs Service Panel - Employees accessing lathe were exposed to electrocution hazards from the cover missing on the upstairs service panel, on or about June 29th 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
