29 CFR 1910.107(b)(5)(i):  Electrostatic spraying operations were conducted when the average air velocity over the open face of the booth was less than 60 linear feet per minute.    On or about August 20, 2018, in Paint B, during powder coating operations using Nordson Excel 2000 Powder Systems Model 2003H, serial number 81-098-190, the average air velocity over the open face of the employee alcove on the right side was 53.78 linear feet per minute.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate protective eye equipment when exposed to eye hazards where there was a reasonable probability of injury that could be prevented by such equipment.  On or about August 21, 2018, in Paint B, the employer did not require painters to use chemical splash goggles when working inside the Nordson Excel 2000 Powder Coating System during spraying and cleaning activities. Employees were exposed to eye hazards from flying particles that could result in eye irritation, redness and swelling.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  On or about August 21, 2018, the employer did not ensure that respirators were stored to protect them from damage, contamination and dust in that respirators were stored in metal boxes without protective coverings. Employees were required to wear respirators during activities such as, but not limited to spraying powder coating and cleaning the spray booth.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about July 2, 2018, the employer did not provide effective training on hazardous chemicals such as, but not limited calcium carbonate and carbon black, which are included in Sherwin-Williams POWDURA Hybrid Powder Coating- PNT1101D Black and used by paint department employees when coating metal parts. Employees were exposed to inhalation, skin and eye contact hazards that could result in health effects such as, but not limited to eye, skin and respiratory irritation.
