29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) 3925 North Front Street Harrisburg PA, - Employees were observed performing roofing activity on a low-slope roof at Wendy's, without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 14 feet 2 inches to the ground below, on or about July 2, 2018. No abatement certification or documentation required.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: (a) 3925 North Front Street Harrisburg PA- The employees were using a 8 foot yellow fiberglass step ladder in the closed position leaning up against the back wall to access the fixed steel ladder attached to the back wall to access the roof, exposing the employees to injuries from ladder collapse or deflection, on or about July 2, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. .
