29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) On or about June 30, 2018, the employer did not ensure that appropriate eye or face protection was used by employees when exposed to eye or face hazards from flying particles. Employees were exposed to eye and face hazards from flying particles in that they did not use eye or face protective equipment while operating pneumatic nail guns while conducting framing activities on a residential structure. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) On or about June 30, 2018, the employer did not ensure that each employee(s) engaged in residential construction activities 6 feet or more above lower levels were protected by guardrails, safety nets, or personal fall arrest systems. Employees of Pecas Construction, LLC. were exposed to fall hazards of up to 13 feet, while engaged in residential framing activities. Employees were not using fall protection equipment. 29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    (a) On or about June 30, 2018, the employer did not ensure that stairways or ladders were provided at all personnel points of access where there was a break in elevation of 19 inches or more. Employees were exposed to falls of up to 13 feet while working on the top plate of a residential building under construction. Employees were observed climbing down wooden trusses to reach the ground instead of using ladders.
