29 CFR 1910.119(c)(2): The employer did not consult with employees and their representatives on the conduct and development of process hazards analyses and on the development of the other elements of process safety management in this standard.    a) The employer is failing to protect employees from fire/explosion hazards while preparing fireworks for display. This was most recently documented on July 3, 2018 at A.M. Pyrotechnics LLC located at 2429 E. 535th Road in Buffalo, MO. The employer did not consult with employees on the conduct and development of process hazard analysis in the pre-display assembly of fireworks  and on the development of the other elements of process safety management in this standard.    b) The employer is failing to protect employees from fire or explosion hazards while preparing fireworks for display.  This was most recently documented on or about July 3, 2018 at A.M. Pyrotechnic Displays LLC, located at 2429 E. 535th Road, Buffalo, MO.  The employer did not consult with employees on the conduct and development of process hazard analysis in the manufacturing of fireworks and development of the other elements of process safety management in this standard.

This was most recently documented on July 3, 2018 at A.M. Pyrotechnics LLC, located at 2429 E. 535th Road in Buffalo, MO.  The employer did not complete a compilation of written process safety information pertaining to:           1) The process of assembling multiple components of display firework devices consisting of Hazard Class/Division 1.3G explosives.            2) The codes and standards being followed to identify the hazards associated with static electricity, stray current and other sources that could affect firework components.            3) The codes and standards being followed to identify the Net Explosive Weight limits and the proper separation distances of the fireworks processing and e-matching area from other activities and personnel within the same operating building.       b) The employer is failing to protect employees from fire and explosion hazards. This was most recently documented on or about July 3, 2018 at A.M. Pyrotechnic Displays LLC, located at 2429 E. 535th Road, Buffalo, MO.  The employer did not complete a compilation of written process safety information pertaining to:           1) The codes and standards being followed to identify the hazards associated with assembling multiple components of display firework devices. which included but are not limited to star rolling, comet pressing, and shell assembly. Pyrotechnics LLC, located at 2429 E. 535th Road in Buffalo, MO when the employer did not train or presently involve employees in the operating process, and each employee before being involved in operating a newly assigned process. In addition employees were not trained on any safety and health hazards, emergency operations including shutdown, and safe work practices that were applicable to the employee's job tasks.    b) The employer is failing to protect employees from fire and explosion hazards. This was most recently documented on or about July 3, 2018 at A.M. Pyrotechnic Displays LLC, located at 2429 E. 535th Road, Buffalo, MO. This was most recently documented on July 3, 2018 at A.M. Pyrotechnics LLC, located at 2429 E. 535th Road in Buffalo, MO. The employer did not establish an emergency action plan to ensure evacuation and aid was available for injured employees in the event of an accident that may occur involving Hazard Class/Division 1.3G explosives.     b) The employer is failing to protect employees from fire and explosion hazards This was most recently documented on or about July 3, 2018 at A.M.
