29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about June 27, 2018, employees operating the Spadone Cutters in Banbury were exposed to amputation hazards when accessing the area behind the blade(s) to place or retrieve material: a. The #1 guillotine cutter was not guarded on the back side of the machine. b. The #2 guillotine cutter was not guarded on the back side of the machine. c. The #3 guillotine cutter was not guarded on the back side of the machine. d. The #4 guillotine cutter was not guarded on the back side of the machine.
