29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. On July 6, 2018, an employee was exposed to falling from a personnel platform supported by a JCB telehandler when the guardrails had not been put in place to provide fall protection.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On and about July 6, 2018, employees were exposed to falls from the second floor deck to the ground while framing a gable end wall when the guardrail protecting the edge had been removed and no other fall protection was established.

Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace. 29 CFR 1910.178(l)(3)(i)(G): Fork and attachment adaptation, operation, and use limitations; On and about July 6, 2018, an employee was exposed to falling from a forklift supported personnel platform when the operator had not been trained to understand and follow the applicable standards contained in ANSI B56-6, in that: A: The operator was moving the lift while it was occupied; B: The platform was not marked with its weight or capacity.
