29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000: LATHE DEPARTMENT, RIGHT BAY: On or about July 5, 2018, a Hyster Model S120XMS-PRS forklift was operated indoors while three employees were moving equipment around. After working for approximately six hours, all three employees were experiencing symptoms of carbon monoxide (CO) poisoning. CO in the exhaust from the forklift was measured at concentrations between 750ppm and 1,052ppm.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts carbon monoxide per million parts air (50 ppm): LATHE DEPARTMENT, RIGHT BAY: On or about July 5, 2018, three employees were using a fork truck to move a CNC machine out of the building when they were exposed to carbon monoxide at concentrations in excess of the 8-hour Time Weighted Average (TWA) Permissible Exposure Limit (PEL) of 50 parts per million (ppm). The fork truck was being used inside the building with doors closed thereby limiting the amount of ventilation. a) One employee had been exposed to carbon monoxide at an 8-hour TWA of 483.6 ppm which is 9.67 the PEL. The exposure concentration was derived by entering the employee's carboxyhemoglobin (COHb) concentration and other site-specific data into the Coburn-Forster-Kane equation. b) A second employee had been exposed to carbon monoxide at an 8-hour TWA of 142.4 ppm which is 2.85 the PEL. The exposure concentration was derived by entering the employee's carboxyhemoglobin (COHb) concentration and other site-specific data into the Coburn-Forster-Kane equation.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: COMPANY-WIDE: On or about and at times prior to July 5, 2018, the company's Hyster Model S120XMS-PRS Forklift used 60-inch fork extensions. The data plates had not been marked to identify the attachments, the approximate weight of the truck, and the down-rated capacity of the vehicle with the extensions in use.
