29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: UNIT 6 - TARGET ROOM: On or about and at times prior to April 29, 2019, employees working in the second floor "Target Room" were exposed to fall hazards. There was a "jump door" that was open at times to provide ventilation and to received deliveries. There was no guardrail to prevent employees from falling.

29 CFR 1910.28(b)(2)(ii): The employer did not ensure that when any portion of a guardrail system, gate, or chains was removed, and an employee must lean through or over the edge of the access opening to facilitate hoisting, employees were protected from falling by a personal fall arrest system: UNIT 6 - TARGET ROOM: On or about and at times prior to April 29, 2019, when deliveries were received to the second floor Target Room, employees were required to stand at the "jump door" to guide the delivery into the building. Employees were not provided with fall protection to prevent them from falling 10 feet to the ground.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: COMPANY-WIDE: On or about and at times prior to May 13, 2019, the employer did not ensure that employees were aware of the hazards associated with chemical products that they used regularly. Hazardous chemicals used at the facility include but are not limited to potassium hydroxide-based cleaners.
