29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye; (a) 1532 Commerce Avenue - An employee was hospitalized from a broken tibia and ankle when the walk-rider they were operating backed into a guard rail, on June 15, 2018. The employer failed to report this work-related amputation to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on June 18, 2018.)
