29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) On or about 06/15/2018 - Facility, employees operated a Toyota 1400 (Model #7FDU45, S/N #70509 ) powered industrial truck with broken mirrors, horn, and a damaged seat. b) On or about 06/15/2018 - Facility, employees operated a Hyster 80XL (Model #S30XLBCS, S/N #004D01985T) powered industrial truck with an inoperable horn. c) On or about 06/15/2018 - Facility, employees operated a Hyster LP powered industrial truck (Model #S12OXMS-PRS, S/N #FGV4V01986B) with a broken bracket that secures the LP tank on the back of the truck. d) On or about 06/15/2018 - Facility, employees operated a Caterpillar (Model #V330B, S/N # 72401507) powered industrial truck with a damaged operator windshield and an inoperable horn.
