29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On July 3, 2018, located on the north side of an interior commercial structure, an employee was utilizing an extension ladder to gain access to the top floor mezzanine as there were no frequent and regular inspections being performed by a competent person such as but not limited to ensuring ladders were not used on slippery surfaces, unleveled surfaces and securing the ladder to prevent accidental displacement prior to use. Potential fall height measured to be 10'-7". In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer failed to report a work-related inpatient hospitalization accident within 24 hours of learning of an employee who fell from an extension ladder while performing drywall work on July 3, 2018 at site address 6905 Given Rd., Cincinnati, OH 45243. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
