On or about July 10 2018, an employee operated a concrete cut off saw with the banjo bolt water attachment nozzle and wheel guard limit stop, to allow the saw operator to rotate the wheel guard back further into the handle, exposing the top front 1/4 of the segmented diamond concrete saw blade, exposing the employee to struck by hazards of the exposed saw blade kick back.
