On or about December 18, 2025, on the corner of 16th Avenue North & 1st Street North in Jacksonville Beach Florida, in a 5 foot 4 inch deep trench, the employer did not provide a stairway, ladder, ramp or other safe means of egress, exposing employees to emergency egress hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about December 18, 2025, on the corner of 16th Avenue North & 1st Street North in Jacksonville Beach, Florida, employees working in a 5 foot 4 inch deep trench were not protected from soil cave-in and engulfment hazards in that they were not protected by a protective system.
