29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) On July 10, 2018, the employer did not ensure employees on top of a 13 foot 3 inch high flat roof were utilizing fall protection. The job activity was to replace an existing flat roof on a commercial building located at 14709 Ridge Rd. North Royalton, OH 44133.    b) On July 10, 2018, the employer did not ensure employees on top of a 32 foot 8 inch high flat roof were utilizing fall protection. The job activity was to replace an existing flat roof on a commercial building located at 14709 Ridge Rd. North Royalton, OH 44133.    Professional Roofing Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1309135, citation number 1, item number 1 and was affirmed as a final order on 07/05/2018, with respect to a workplace located at 8787 Rockside Rd. Independence, OH 44131.

29 CFR 1926.502(h)(1)(iii):  The safety monitor shall be on the walking/working surface and within visual sighting distance of the employee being monitored:    On July 10, 2018, the employer did not protect employees from fall hazards of approximately 13 foot 3 inch high on a flat roof by ensuring that the safety monitor was on the same walking/working surface and within sighting distance of the employees being monitored.The job activity was to replace an existing flat roof on a commercial building located at 14709 Ridge Rd.
