29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Machine Shop: On or about June 19, 2018 and times prior, the employer did not ensure that machine guarding was provided on an Ellis Manufacturing, Inc. drill press to protect employees from the rotating chuck. (b) Machine Shop: On or about June 26, 2018, the employer did not ensure that machine guarding was provided on a Cincinnati lathe to protect employees from the rotating chuck.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Machine Shop: On July 10, 2018, an employee used a pedestal grinder (Model 4Z911) that did not have a tongue guard allowing for a 2 3/4 inch opening between the wheel and housing.
