29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation: (a) 2019 OSHA Form 300: On October 16, 2019 and times prior, an employee injury occurred on 05/20/2019 that resulted in the employee missing 83 days of work. The employer did not provide information in column F of the 2019 OSHA Form 300 that explained the injury resulted in an amputation. The employer also did not classify the injury as "days away from work" or include the number of days away from work on the 2019 OSHA Form 300 as required per 29 CFR 1904.7(b)(3).
