The spliced area exposed approximately 4 inches of inner conductors.

The employee was setting trusses on the east side of Townhouse A. (b) Employees engaged in framing a residential four-plex building located at 23921 W. 66th Street, Lot 19, Shawnee, Kansas were exposed to a fall hazard of approximately 9 feet due to no fall protection system being provided in Townhouse A. The employees were framing the lower level of Townhouse A next to an open stairway that was approximately 4 feet wide by 9 feet long. (c) Employees engaged in framing a residential four-plex building located at 23921 W. 66th Street, Lot 19, Shawnee, Kansas were exposed to a fall hazard of approximately 9 feet due to no fall protection system being provided in Townhouse B. The employee was erecting a scaffold on the north side of townhouse B next to an open stairway that was approximately 4 feet wide by 9 feet long. (d) Employees engaged in framing a residential four-plex building located at 23921 W. 66th Street, Lot 19, Shawnee, Kansas were exposed to a fall hazard of approximately 9 feet due to no fall protection system being provided in Townhouse C. The employee was standing on floor joist preparing to frame the next level of the building on the northeast side of Townhouse C. (e) Employees engaged in framing a residential four-plex building located at 23921 W. 66th Street, Lot 19, Shawnee, Kansas were exposed to a fall hazard of approximately 9 feet due to no fall protection system being provided in Townhouse C. The employees were framing the lower level of Townhouse C next to an open stairway that was approximately 4 feet wide by 9 feet long. (f) Employees engaged in framing a residential four-plex building located at 23921 W. 66th Street, Lot 19, Shawnee, Kansas were exposed to a fall hazard of approximately 9 feet due to no fall protection system being provided in Townhouse D. The employees were framing the lower level of Townhouse D next to an open stairway that was approximately 4 feet wide by 9 feet long. The D and D Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1302067, citation number 1, item number 2 and was affirmed as a final order on June 19, 2018, with respect to a workplace located at 4721 NE Pocono Circle, Lot 39, Lees Summit, Missouri.
