29 CFR 1926.651(b)(1): The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, shall be determined prior to opening an excavation.    On July 11, 2018, the employer failed to control gas utility locate and exposed employees to potential fire and explosion hazards, in that while excavating an area for the installation of new storm sewer pipes a gas utility was struck.  The gas utility had been previously located had been removed during dig and was not reestablished before digging resumed.
