29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Northern Illinois University, Holmes Student Center - On or about July 12, 2018, employees performing asbestos remediation operations from a mobile scaffold were exposed to falling in excess of 10 feet when the employer failed to ensure a guardrail system was installed around the working level. No abatement certification or documentation is required for this item.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: Northern Illinois University, Holmes Student Center - On or about July 12, 2018, employees performing asbestos remediation operations from a mobile scaffold were exposed to falling in excess of 10 feet when the employer failed to ensure that the casters (wheels) were locked to prevent movement of the scaffold while employees climbed to and worked at the upper level. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
