On or about July 12, 2018 - at the above addressed jobsite,  a JLG 400S Telescopic Aerial Boom Lift was used to hoist a steel beam using a cable wire sling which was affixed to the bottom of the basket.  An employee was exposed to hazards associated with overloading and failure of the lift.
