29 CFR 1926.501(b)(13): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system. On or about July 10, 2018, employees engaged in roofing operations were exposed to fall hazards in excess of 6-feet without using any form of fall protection. Luis Hernandez was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13) which was contained in OSHA inspection number 1211604, citation number 1, item number 1 and was affirmed as a final order on 04/14/2017, with respect to a workplace located at7792 Enchanted Trail Dr., El Paso, TX 79911.

29 CFR 1926.503(c)(3): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section. On or about September 10, 2018, employees engaged in roofing operations were exposed to fall hazards in excess of 6-feet due to inadequacies in their fall protection training. Luis Hernandez was previously cited for a violation of this occupational safety and health standard 1926.503(c)(3) which was contained in OSHA inspection number 1211604, citation number 1, item number 2 and was affirmed as a final order on 04/14/2017, with respect to a workplace located at7792 Enchanted Trail Dr., El Paso, TX 79911.
