Instance a)  4 Nast Road, Building #48, Livingston, NJ.    Subcontractor employees were exposed to falls from the 40' portable aluminum extension ladder used to access upper levels given that the ladder was not withdrawn from service or marked as defective as rails were bent, a rung was bent and a rung and ladder feet were missing.                                     Violation observed on or about 07/03/2018 and on 07/05/2018.
