29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) 536-526 Byler Circle, Lebanon PA - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building and were exposed to flying debris and ricochet nails, on or about July 10, 2018. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, and the ladder was secured at its top to a rigid support that will not deflect, a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) 536-526 Byler Circle, Lebanon PA - Employees used a portable Werner extension ladder to access the roof and the side rails were extended to its maximum length and was inches above the landing surface. A grasping device was not provided to assist the employees in mounting and dismounting the ladder, on or about July 10, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
