29 CFR 1926.501(b)(4)(ii): Each employee on walking/working surfaces was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about July 17, 2018, at a new building construction site located on the corner of N.W. 15th and Penn in Edmond, OK, employees of a subcontractor were allowed to work in close proximity to uncovered holes at the site, exposing the employee to the hazard of falls.
