29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    On July 10,2018, in the Sheet Metal Shop Area of the facility located in Cassville, MO., an employee was exposed to serious crushing injuries to the hands and fingers, in that the fixed guard on the Hydraulic Shear was not affixed in a manner to prevent injuries to the operators. An employee was able to place his fingers under the guard and into the danger zone of the cycling hold down clamp assembly of the machine.  As a result, his left middle finger was badly crushed and resulted in a partial amputation.
