29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 445 Catherine Street - Front of the Building South west: Employees were exposed to fall hazards of approximately 12' 6" to the ground below while positioned at the unprotected edge of stair landing in which a means of fall protection was not utilized, on or about 7/19/2018.   b) 443 Catherine street - Front of the Building South East: Employees were exposed to fall hazards of approximately 10' to the ground below while positioned at the unprotected edge of stair landing in which a means of fall protection was not utilized, on or about 7/19/2018.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.  a) 445 Catherine street - south west, on scaffold: Employee was exposed to fall hazards of approximately 7 feet while working on a fabricated framed scaffold that was not fully planked, on or about 7/19/2017   The Milanos  Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1):, which was contained in OSHA inspection number 1076969, citation number 1, item number 1 and was affirmed as a final order on 9/29/2015, with respect to a workplace located at 65 Columbia Ave Cliffside Park NJ 07010.
