Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 6/1/2018, employees of AF Construction LLC were working on the roof of a residential building located at 1 Fairway Oaks Court, Lake Saint Louis, MO. Employees were engaged in roofing work approximately 10 feet above a lower level. Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

1926.502(d)(21): Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service. On 6/1/2018, employees of AF Construction LLC were working on the roof of a residential building located at 1 Fairway Oaks Court, Lake Saint Louis, MO. Employees were engaged in roofing work approximately 10 feet above a lower level. The fall protection equipment was not inspected for wear, damage or deterioration prior to use. The employees were observed on the roof using damaged equipment exposing themselves to a fall to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards. On 6/1/2018, employees from AF Construction LLC were performing roofing operations at 1 Fairway Oaks Court, Lake Saint Louis, MO. The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards. Employees were wearing body belt harnesses (Black Diamond and Petzl) designed for rock climbing and recreational use only. Ropes had excessive abrasions and cuts exposing employees to a fall to the ground below.
