29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or ace protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Badillo Roofing Inc. Located at 3026 Crusader Street, Fort Collins, CO 80524: On and preceding 7/21/2018, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. Two employees were installing shingles on a commercial roof using pneumatic nail guns without the use of eye protection. This condition exposed the employees to struck-by injuries to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Badillo Roofing Inc. Located at 3026 Crusader Street, Fort Collins, CO 80524: On and preceding 7/21/2018, the employer did not ensure that employees engaged in residential roofing activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems. Two employees were installing shingles and trim on the roof of a two-story residential home without the use of fall protection. This exposed employees to fall hazards from height.
