29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Rapid Pump & Meter Co., Inc./Linden, NJ- Capital Foods (Loading Dock Area): The employer failed to develop procedures for the control of potentially hazardous energy. Technicians required to perform maintenance/servicing on the equipment such as but not limited to pumps, were not provided with procedures to follow in order to isolate all points where hazardous energy could be stored. Steps to follow to relieve hazardous residual chemical energy contained in the vessel/pump and piping system prior to conducting work on the equipment were not provided. A technician who was servicing a pump on the sewer discharge system was splashed with solution containing sodium hydroxide and suffered chemical burns. Condition occurred on or about May 1, 2018.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Rapid Pump & Meter Co., Inc./Linden, NJ- Capital Foods (Loading Dock Area): The employer did not ensure that technicians required to perform maintenance/servicing on the equipment including but not limited to pumps, discussed with the on-site employer (Capital Foods) their respective procedures and control measures to be used, for all hazardous energy sources potentially to be encountered. A technician who was servicing a pump on the sewer discharge system did not identify and isolate all points in order to dissipate residual chemical energy and was splashed with solution containing sodium hydroxide. Condition occurred on or about May 1, 2018.
