Left hand side of building parapet clamp was tied back by a single line approximately 30 degrees from the perpendicular; on or about 7/13/18.    b). Right hand side of building parapet clamp was tied back by a single line approximately 45 degrees from the perpendicular; on or about 7/13/18.

The suspended scaffold had multiple deficiencies such as but not limited to, no guardrails, improper tie backs and broken components.  The suspended scaffold was not inspected by a competent person before each use; on or about 7/13/18.
