Dolgencorp LLC dba Dollar General #4903, Toilet Facilities area, on or about July 24th, 2018 and times prior thereto: Human waste was located in the toilet facilities in the employers toilet rooms. The toilet facilities were in need of repair in that, they could not be flushed properly.
