29 CFR 1926.757(e)(5): When construction loads were placed on steel joists, the employer did not ensure that the load was within 1 foot of the bearing surface of the joist end. Site- Employees were not protected from fall, struck by, and crushing injuries where bundles of decking were placed more than one foot and up to approximately 10 feet away from the bearing end of open web steel roof joists that were not anchored at each joist bearing end and prior to all bridging being installed and anchored.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Site- On or about July 20, 2018 an employee sustained a serious fall injury which required an in-patient hospitalization of the employee and the in-patient hospitalization was not reported by the employer to OSHA within 24 hours.
