29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Employee(s) were observed installing roof shingles atop 8 Ashley Road in Edison NJ without personal fall arrest equipment or any other means of fall protection; exposing them to 17 ft fall hazard.     Condition(s) were noted on or about 7/20/2018.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Employee(s) used a Werner extension ladder for access to / from the roof of a single-family house under construction at 8 Ashley Road in Edison, where roof shingles were being installed.  The ladder side rails did not extend at least 3 ft above the upper landing of the roof as a grasping point at the top of the ladder; exposing employee(s) to hazard of falling 17 ft to the ground below.    Condition(s) were noted on or about 7/20/2018.
