29 CFR 1926.652(a)(1): Employees were not protected in an excavation from a cave-in by an adequate protective system designed in accordance with paragraph (b) and (c) of this section. (a) At the construction site located near N145th St and Capitol Drive, Brookfield, WI, employees were performing work in an approximately 8 5/10 feet deep excavation without adequate cave-in protection, thereby exposing employees to a struck-by / crushing hazard.
