29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  The employer allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections.  The most recent occurrence was at 12423 Questover Manor Court, Creve Coeur, Missouri 63141, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with OSHA standards.   The workers were exposed to a fall hazard ranging from approximately 13 feet as they were performing residential roofing activities on an approximate 8:12-10:12 pitch without the use of any fall protection system.  In addition, employees failed to wear eye protection while operating nail guns to perform roofing operations.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer's training program did not enable each employee to recognize the hazards of falling at 12423 Questover Manor Court, Creve Coeur, Missouri and did not train each employee in the procedures to be followed in order to minimize the hazards of falling during roofing activities.

Martha Soto was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1109448, citation number 1, item number 1 and was affirmed as a final order on 12/28/2015, with respect to a workplace located at 1939 Lake Clay Drive, Chesterfield, MO.

Martha Soto was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1109448, citation number 1, item number 2 and was affirmed as a final order on 12/28/2015, with respect to a workplace located at 1939 Lake Clay Drive, Chesterfield, MO.
