Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 7/25/2018, C. Mac Construction, Inc. was installing sheathing on a roof of a residential building located on Lot 688 Mule Creek Drive in Wentzville, MO. The foreman and an employee were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. They were observed on a 6/12 sloped roof of the building approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards. On 7/25/2018, C. Mac Construction, Inc. was installing sheathing on a roof of a residential building located on Lot 688 Mule Creek Drive in Wentzville, MO. The foreman and an employee were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.
