On or about July 30, 2018, 11 Thompson Road, East Windsor, CT rooftop jobsite:  The Amerex ABC portable fire extinguisher available for use by employees using flammable materials had been discharged and not replaced with a fully charged extinguisher. Employees working at the site were exposed to flammables, including, but not limited to: gasoline, acetone, Weathered Membrane Cleaner containing naphtha, and Sarnacol 2170 VC Membrane Adhesive containing butanone, toluene, xylene and ethylbenzene.

On or about July 30, 2018, 11 Thompson Road, East Windsor, CT rooftop jobsite:  Where more than 5 gallons of flammable liquids, including gasoline, acetone, Weathered Membrane Cleaner containing naphtha and Sarnacol 2170 VC Membrane Adhesive conaining butanone, toluene, xylene and ethylbenzene were used, no fire extinguishing equipment was provided within 50 feet.  One discharged Amerex ABC portable fire extinguisher was present on the roof.

On or about July 30, 2018, 11 Thompson Road, East Windsor, CT rooftop jobsite:  Mechanical equipment including a cutter and four-wheeled carts were used on the rooftop jobsite.  Warning lines at the parking lot side of the roof were attached to the guardrail system, located in 31 inches from the roof edge.
