29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: Phase Dock: On or about July 25, 2018 and at times prior thereto, a slippery petroleum substance was allowed to spill on the walking surface of the hangar floor. This condition exposed employees to slip hazards.

29 CFR 1910.134(i)(1)(ii): Breathing air did not meet requirements for Grade D breathing air as described in the ANSI/Compressed Gas Association Commodity Specification G-7.1-1989: Sheet Metal Shop: On or about July 25, 2018 and at times prior thereto, the employer could not confirm that breathing air for supplied-air respiratory protection could meet Grade D requirements. This condition exposed employees to inhalation hazards in the event of inadequate supplied breathing air.
