29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about July 25, 2018 and at times prior, an employee was exposed to fall hazards of 8 feet while working on the roof of a south facing residential house. The rope was installed on the west side of the roof while the employee was working on the east side. The rope used on the personal fall arrest system had excessive horizontal length that would not stop the employee in the event of a fall.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet: a) On or about July 25, 2018 and at times prior, two employees were exposed to fall hazards of 8 feet while using an extension ladder to access the roof of a residential house. The side rail of the ladder was not extended at least 3 feet above the landing surface.
