29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) 2102 W. Great Neck Rd, Virginia Beach, VA 23451: The employer did not provide employees with the information contained in Appendix D, where employees were permitted to wear N95 and half-face elastomeric negative pressure respirators on a voluntary basis, exposing employees to potential respiratory irritation. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Great Neck Rd, Virginia Beach, VA 23451: The employer did not document a hazard assessment for employees that worked with hazardous chemicals that required use of PPE, including but not limited to, impermeable and/or chemical resistant gloves, acid-resistant protective clothing, protective eyewear including safety glasses with sideshields, goggles, and/or face shields, exposing employees to potential eye and skin irritation or sensitization. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
