29 CFR 1926.451(b)(5)(i): The end of the platform 10 feet or less in length extended over its support more than 12 inches, and was not designed and installed so that the cantilevered portion of the platform was able to support employees and/or materials without tipping, or had guardrails which blocked employee access to the cantilivered end.  a) Jobsite: Carlton Street side scaffold, 1st working platform, north side - The cantilevered portion of the platform on the supported tubular welded frame scaffold extended more than 12 inches over its support exposing employees to tipping hazards.  Employees were performing bricklaying operations from the outrigger above the 1st working platform, and used the 1st working platform, approximately 20 feet above ground level, as an access point.  Observed on or about 7/31/18.

29 CFR 1926.451(b)(8): At all points of a scaffold where the platform changes direction, such as turning a corner, the platform that rested on a bearer at an angle other than a right angle was not laid first, and the platform which rested at a right angle over the same bearer was not laid second, on top of the first platform.  a) Jobsite: 17th St. scaffold, between 3rd and 4th working platforms, and Carlton St. scaffold, between the 1st and 2nd working platforms  - The platform that changed direction around the corner of the building did not rest on bearers.  Observed on or about7/31/18.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) Jobsite: Carlton St. side scaffold, balcony - The employer did not provide a safe means of access from one level of the tubular supported welded frame scaffold to another by the use of portable ladders, hook-on ladders or other acceptable means . Employees climbed from the balcony level up to the 1st working platform, then approximately 3 feet up to the outrigger over the 1st working platform, using the end-frames and cross braces as support.  Observed on or about 7/31/18.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite: 17th Street scaffold, outrigger between 3rd and 4th level platforms, north end - Employees conducting masonry operations from the outrigger of the supported tubular welded frame scaffold were exposed to fall hazards of approximately 23 feet above ground level without the use of an adequate fall protection system.  b) Jobsite: Carlton Street side scaffold, outrigger between 1st and 2nd level platforms, north end: Employees conducting masonry operations from the outrigger of the supported tubular welded frame scaffold were exposed to fall hazards of approximately 23 feet above ground level without the use of a fall protection system.  c) Jobsite: Carlton St. side scaffold, 2nd level end-frame, 1st working platform, north end:  Employees conducting masonry operations from the supported tubular welded frame scaffold were exposed to fall hazards of approximately 20 feet above ground level without the use of a fall protection system.  d) Jobsite: 17th St. scaffold, 5th level end-frame, 4th working platform, north end: Employees conducting masonry operations from the supported tubular welded frame scaffold were exposed to fall hazards of approximately 26 feet above ground level without the use of a fall protection system.  e) Jobsite: Walking/working platform connected between the Carlton St. side scaffold, and the 17th St. scaffold on north side of north west corner of building - Employees conducting bricklaying operations from the walking/working platform, located approximately 23 ft. above ground level, were exposed to the fall hazard without the use of a fall protection system.   The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1243258, citation number 1, item number 2a, and was affirmed as a final order on 12/11/17, with respect to a workplace located at631 N. Broad Street, Philadelphia, PA. 19123.   Observed on or about 7/31/18.
