29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    Y & T Layout located at 4682 Hahns Peak Drive, Loveland, CO 80534: On and preceding 1 August 2018, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles to include but not limited to debris from use of pneumatic nail guns.  Employees installing trusses with a pneumatic nail gun on a residential roof were not utilizing eye or face protection.  This exposed the employees to struck by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Y & T Layout located at 4682 Hahns Peak Drive, Loveland, CO 80534: On and preceding 1 August 2018,  The employer did not ensure that employees engaged in residential framing activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems.  Two employees were installing trusses on a three story residential building without the personal fall arrest system lanyard connected to their harnesses while they were actively engaged in framing work.  The exposed employees to falls from height.

29 CFR 1926.502(d)(6)(i): Snap hooks were connected directly to webbing, rope or wire rope:  Y & T Layout located at 4682 Hahns Peak Drive, Loveland, CO 80534: On and preceding 1 August 2018, the employer did not ensure that snap hooks were not connected directly to rope used for anchorage of a rope grab fall protection system.  Framers were engaged in residential framing activities 6 feet or more above lower levels and the personal fall arrest system snap hook was attached directly to the ropes on all four setups.  Four employees were installing trusses on a three story residential building while using these rope grabs and ropes.  The exposed employees to falls from height.

Framers were engaged in residential framing activities 6 feet or more above lower levels and the personal fall arrest system snap hook was attached directly to the ropes on all four setups.  Four employees were installing trusses on a three story residential building while using these rope grabs and ropes.  Additionally, all four of the ropes and lanyards were connected at or below foot level of the framers. The exposed employees to falls from height.

Four employees were installing trusses on a three story residential building which they used an 8' A-frame ladder to access the upper landing which was 9' or more.  The exposed employees to falls from height.

Y & T Layout located at 4682 Hahns Peak Drive, Loveland, CO 80534: On and preceding 1 August 2018, the employer did not ensure that personal protective equipment was provided to framer who were engaged in activities which require the use of eye and head protection.  Four employees were installing trusses on a three story residential building which they used pneumatic nail gun and worked at different levels which increased the risk of struck-by hazards.   The employer did not provide personal protective equipment to the employees.  The employer required the employees to provide their own eye and head protection.
