29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) Y&T Layout located at 11250 Florence Street, Henderson, CO 80640: On or preceding October 16, 2019, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles when using pneumatic nail guns. Employees installing trusses and sheathing with pneumatic nail guns on a residential roof were not utilizing eye protection. This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Y&T Layout located at 11250 Florence St, Henderson, CO 80640: On and preceding October 16, 2019, the employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize the hazards. Framers were engaged in residential framing activities 6 feet or more above lower levels without fall protection training. Two employees were installing trusses and sheathing on a three story residential building, working on the outer edges with no fall protection at heights of 20-25 feet. This condition exposed employees to falls from height.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Y&T Layout located at 11250 Florence St, Henderson, CO 80640: On or preceding October 16, 2019, the employer did not ensure that employees engaged in residential framing activities 6 feet or more about lower levels was protected by guardrail systems, safety net systems, or personal fall arrest systems. Two employees were installing trusses and sheathing on a three story residential building without the use of fall protection, and were actively engaged in framing activities on the edge of the building. This condition exposed employees to fall hazards of approximately 20-25 feet. Y&T Layout was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1335019, Citation Number 1, Item Number 2a and was affirmed as a final order on September 18, 2018, with respect to a workplace located at 4682 Hahns Peak Drive, Loveland, CO 80534.
