29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) EVIDENCE PROCESSSING ROOM: Employees who worked with hazardous chemicals including, but not limited to, reagents used to determine the presence of Fentanyl and other opiates, were not provided with suitable facilities for eye flushing within the work area for immediate emergency use. (b )FORKLIFT CHARGING STATION: Employees who handle forklift batteries containing sulfuric acid (corrosive chemical) were not provided with a suitable eye and body wash station, exposing them to injury from corrosive materials.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) GEMINI ROOM: The outer jacket on the extension cord powering the PurAir portable ventilation hood had visible damage, exposing employees to electrical shock and fire hazards.
