29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about July 27, 2018, the employer failed to inspect the job site and educate each employee on the possible health and safety hazards that were or may be present nor did the employer make an effort to control or eliminate any recognized hazards.  The job site consisted of a commercial roofing project (several buildings which were connected-Christ Church Episcopal, 21 Aurora St., Hudson OH).  Employees were exposed, including but not limited to, falls from height (30+ feet), eye and head injuries.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about July 27, 2018, the employer failed to ensure that employees were protected from hazards at the job site (Christ Church Episcopal, 21 Aurora St., Hudson OH) to include but not limited to, head injuries.  Employees are exposed to debris that is either falling or being thrown off the roof top of a commercial building from varied heights exceeding 30 feet.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about July 27, 2018, the employer failed to ensure that employees were protected from hazards at the job site (Christ Church Episcopal, 21 Aurora St., Hudson OH) to include but not limited to, eye injuries.  Employees are working with nail and staple guns and were not wearing any protective safety eyewear.

29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, shall be used:    On or about July 27, 2018, the employer failed to ensure that an enclosed chute was in use where employees were throwing roofing shingles and other materials off of a church at a job site (Christ Church Episcopal, 21 Aurora St., Hudson OH) that they were re-roofing.  There was roofing debris, wood pieces and other metallic debris on the ground all around the churches exterior where the work above was being performed.   Employees were not wearing hard hats and were exposed to falling objects and debris that was being thrown from off the top of the roof (approximately 30-40 feet in height).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about July 27, 2018, the employer failed to ensure that employees who were working on a roof at a job site (Christ Church Episcopal, 21 Aurora St., Hudson OH) were protected from a fall from height hazard of approximately 30-40 feet.  The roof had a pitch of 9/12 and employees were reroofing the entire facility so the heights varied.  Employees were not wearing any personal fall protection harnesses nor was there any alternative fall protection system in place.
