29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about 8/23/18, during residential roofing activities located at 21818 Royalton Road in Strongsville, Ohio, the employer failed to ensure that employees were provided and utilized fall protection. The height of the roof of the home and garage was about 11 feet. The slope of the roof of the home was about 4/12, and the garage slope was about 6/12.
