29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   (a) On or about August 4, 2018, the employer did not ensure that eye and face protection were used by employees when machines or operations presented potential eye or face injury. Employees of Ricardo Reyes were observed nailing shingles on a residential roof using a pneumatic gun without any eye or face protection. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a) On or about August 4, 2018, the employer did not ensure that its employees were protected from falls while working at heights greater than 6 feet. Employees of Ricardo Reyes were exposed to fall hazards of up to 16 feet while engaged in roofing activities on a residential building with no fall protection systems in place.
