29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On July 9, 2018, and times prior there to, throughout the production floor, employees used hoisting chains to conduct material handling lifts, without the employer performing inspections of the hoisting chains.

29 CFR 1910.179(m)(1): Any deterioration, resulting in appreciable loss of original strength, were not being carefully observed and determination made as to whether further use of the rope would constitute a safety hazard: In the Welding area, employees used the overhead bridge crane to move a steel A-Frame. The wire rope/cable for the under-carriage hook had deficiencies such as bird caging, broken and bent wires and had not been taken out of service.
