29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about July 19, 2018, at 10651 NW 88th St, building 8, Summit, Doral, FL, 33178, the employer failed to provide a guardrail systems, safety net system, or Personal Fall Arrest System (PFAS) for an employee engaged in installing roof sheathing 6 feet (1.8 m) or more above lower levels; thus exposing the employee to an approximate 10 to 13 foot fall hazard. BROTHERS CARPENTRY CORP was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR. 1926.501(b)(13), or its equivalent standard, which was contained in OSHA inspection number 1163578, citation number 01, Item number 001, and was affirmed as a final order on 12/29/2016, with respect to a workplace located at 17101 SW 96th Street, Miami, Florida. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On or about July 19, 2018, at 10651 NW 88th St, Doral, FL, 33178, the employer failed to provide employees with training in the use and operation of a Personal Fall Arrest System (PFAS) by a Competent and Qualified person, during which employees were performing construction type work, using a Personal Fall Arrest System (PFAS) at heights above six feet; thus exposing the employee to fall hazards. BROTHERS CARPENTRY CORP was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR. 1926.503(a)(1), or its equivalent standard, which was contained in OSHA inspection number 1163578, citation number 01, Item number 002, and was affirmed as a final order on 12/29/2016, with respect to a workplace located at 17101 SW 96th Street, Miami, Florida.
